Request for enrollment in a course for Dual Credit

 

If you have not filled out an application to become a student at Sandburg, please do so before requesting enrollment in a course for dual credit. Request for course enrollment contingent on approval from high school official. Please allow 48 hours for all dual credit requests.

 By submitting this form, I agree:
·         I will be 16 years of age prior to the start date of the course.
 

 By submitting this form, I understand:
·         It does not guarantee enrollment in the course.
·         Requests for enrollment in a course are on a first-come, first-serve basis.
·         If I do not pay for the course by the payment due date, I will be dropped from the course and will have to do a new request for enrollment in the course, at which point there is no guarantee that a seat in the course is still available.
